- Address of the donor
- Donor identification type and country of issue (for individual donations ID or passport number
- Identification or registration number of the donor (if a company or trust)
- Contact number of the donor
- Electronic mail address of the donor
- Trading name of the donor (Company – if different from the registered name
- Income tax reference number of the donorÂ
